For your first visit, please be sure to arrive 15 minutes before your appointment time and bring the following information with you:

Your Dental insurance card
- Patient Form: Before you come in for your visit with Dr. K, we invite you to fill out your patient form online! We offer an online patient form for your convenience to fast-track your time in the waiting room. (you can complete that form by clicking here).
Insurance and Billing:
*** We file ALL dental insurance claims and will communicate with the insurance company regarding the claims.
*** We do NOT accept DHMO or Medicaid/ Medicare.
